Falls are the leading cause of death in construction and a top killer in general industry. OSHA's fall protection standards exist for one reason: to prevent workers from dying on the job. Yet fall protection violations consistently rank as OSHA's #1 most-cited standard year after year — meaning thousands of employers are still getting it wrong.
This guide breaks down OSHA's fall protection requirements for both construction and general industry, including trigger heights, required systems, training obligations, and what happens when you don't comply.
Two Sets of Rules: Construction vs. General Industry
OSHA maintains separate fall protection standards for construction and general industry. The requirements differ in trigger heights, acceptable systems, and specific obligations. Knowing which standard applies to your operation is the first step.

Construction: 29 CFR 1926 Subpart M
For construction work, OSHA requires fall protection for any worker exposed to a fall of 6 feet or more to a lower level. This applies to:
- Unprotected sides and edges of walking/working surfaces
- Leading edges during active construction
- Excavations 6 feet or deeper
- Ramps, runways, and other walkways
- Dangerous equipment (regardless of height — fall protection required above any dangerous equipment)
- Overhand bricklaying and related work
- Roofing work on low-slope and steep roofs
Acceptable Fall Protection Systems (Construction)
Employers must provide one of the following:
- Guardrail systems — top rail at 42–45 inches, mid-rail at approximately 21 inches, capable of withstanding 200 lbs of force
- Safety net systems — installed as close as practicable below the work surface, no more than 30 feet below
- Personal fall arrest systems (PFAS) — full body harness, connecting subsystem (lanyard or SRL), and anchor point rated for 5,000 lbs per worker
Leading Edge Exception
When conventional fall protection is infeasible or creates a greater hazard at leading edges, employers may use a written fall protection plan with alternative measures. This exception is narrow and requires documented justification — it is not a general exemption from fall protection.
General Industry: 29 CFR 1910 Subpart D
General industry fall protection triggers at 4 feet above a lower level for most walking/working surfaces. Key requirements include:
- 1910.28: Employer duty to provide fall protection — covers unprotected sides and edges, hoist areas, holes, dockboards, runways, dangerous equipment, and more
- 1910.29: Fall protection systems criteria — specifications for guardrails, safety nets, PFAS, covers, and positioning systems
- 1910.30: Training requirements — employers must train workers before they are exposed to fall hazards
Special Trigger Heights (General Industry)
- 4 feet: General walking/working surfaces
- 5 feet: Shipyard employment
- 6 feet: Longshoring operations
- 10 feet: Scaffolds
- Any height: Work over dangerous equipment or machinery
Personal Fall Arrest System (PFAS) Requirements
When a PFAS is the chosen fall protection method, OSHA 1926.502(d) and 1910.140 specify that the system must:
- Use a full body harness (body belts are prohibited for fall arrest since 1998)
- Limit maximum arresting force to 1,800 lbs
- Limit free fall to 6 feet or less
- Bring the worker to a complete stop within 3.5 feet after free fall
- Leave the worker with at least 12 inches of clearance above the lower level after arrest
- Connect to an anchor capable of supporting 5,000 lbs per attached worker, or be part of a certified system designed with a safety factor of 2

Inspection Requirements
OSHA 1926.502(d)(21) requires that personal fall arrest equipment be inspected before each use by a competent person. Equipment must be removed from service if:
- It has been subjected to impact loading (fall arrest event)
- Any component is defective, damaged, or deteriorated
- The manufacturer's service life has been exceeded

Training Requirements
Both construction (1926.503) and general industry (1910.30) require that workers be trained by a qualified person before being exposed to fall hazards. Training must cover:
- The nature of fall hazards in the work area
- The correct procedures for erecting, maintaining, disassembling, and inspecting fall protection systems
- The use and operation of guardrail systems, PFAS, safety nets, and covers
- The role of each employee in the safety monitoring system (where applicable)
- The limitations of mechanical equipment used during roofing work
Retraining is required when the employer believes a worker does not have the understanding or skill required, or when changes in the workplace render previous training obsolete.

OSHA Penalties for Fall Protection Violations

Common Fall Protection Violations to Avoid
- No fall protection at or above the trigger height — the most cited violation; no system in place at all
- Improper use of body belts for fall arrest — prohibited since 1998; body belts may only be used for positioning
- Inadequate anchor points — anchoring to conduit, rebar, or other non-rated structures
- Failure to inspect equipment before use — no competent person inspection, no documentation
- Inadequate training records — training performed but not documented
- Improper lanyard length — using a 6-foot lanyard where clearance below is insufficient to arrest a fall
